After you have received notification of an opening for your child, all forms need to be filled out and brought with you to your designated appointment.

In order to comply with state regulations, all enrollment papers MUST be completed, signed, and on file at Lions N’ Lambs. Required forms are listed below. Click the appropriate link and follow the instructions. If you have additional questions, please contact us at (209) 249–5839.

If your child has an IEP, please bring the IEP paperwork to your Registration appointment.

Preschool Registration (Includes TK & Pre-K)

New Students

  1. Read Preschool Parent Handbook

  2. Complete a New Preschool Registration Packet

  3. Bring all completed forms to your registration appointment along with an updated immunization record.

Returning Students

  1. Read Preschool Parent Handbook

  2. Complete a Returning Preschool Registration Packet

  3. Bring all completed forms to your registration appointment along with an updated immunization record.

Kindergarten Registration

New Students

  2. Complete a New Kindergarten Registration Packet

  3. Bring all completed forms to your registration appointment along with an updated immunization record.

Returning Students

Individual Forms

Our Registration Packets include all necessary forms. There is no need to print the following forms, as they are primarily for office use.
